- Credit Evaluation Form
- Fill out this form if you have credits from other colleges that are being transferred to WVC
- Enrollment Verification (EV) Form
- Required every quarter to continue with Running Start. Fill out your sections and turn in to your high school counselor
- Exit Form
- If you are leaving the program and returning to the high school, fill out this form with your adviser
- Graduation Application
- Ready to graduate? Fill out this form to receive your degree!
- Late Withdrawal Petition
- If you are needing to withdraw from a course past the last day to withdraw due to an extenuating circumstance, fill out this form
- Progress Report
- If you are on probation, please send this form to your professors every 3 weeks and return the completed forms to your adviser
- FERPA: Release of Information
- If you would like to grant permission to a trusted individual to access your WVC information, please fill this out and turn in to Running Start
- Residency Form (in-state tuition charge)
- If you are taking additional credits and being charged tuition, please fill this form out to ensure you are paying the in state rate
- Spring Quarter Eligibility Adjustment Form
- For counselors to use to assist them in calculating remaining FTE for students starting Running Start in Spring Quarter

- If you took a quarter off from Running Start and are returning, fill out this form to have your account reactivated
- Voucher Contract
- If you are an Assist student and want to utilize the $100 book voucher, fill out this form and return to your adviser
