Important Forms for Running Start students

• Credit Evaluation Form 
  • Fill out this form if you have credits from other colleges that are being transferred to WVC
• Enrollment Verification (EV) Form (2026-2027)
  • Required every quarter to continue with Running Start. Fill out your sections and turn in to your high school counselor
• Enrollment Verification (EV) Form for Summer 2026
• Exit Form
  • If you are leaving the program and returning to the high school, fill out this form with your adviser
• Graduation Application
  • Ready to graduate? Fill out this form to receive your degree!
• Late Withdrawal Petition
  • If you are needing to withdraw from a course past the last day to withdraw due to an extenuating circumstance, fill out this form
• Progress Report
  • If you are on probation, please send this form to your professors every 3 weeks and return the completed forms to your adviser
• FERPA: Release of Information
  • If you would like to grant permission to a trusted individual to access your WVC information, please fill this out and turn in to Running Start
• Residency Form (in-state tuition charge)
  • If you are taking additional credits and being charged tuition, please fill this form out to ensure you are paying the in state rate
• Spring Quarter Eligibility Adjustment Form
  • For counselors to use to assist them in calculating remaining FTE for students starting Running Start in Spring Quarter
• Two-year Educational Plan
  • If you would like to create a mock up of a two year plan, download this template

  • If you took a quarter off from Running Start and are returning, fill out this form to have your account reactivated
