COVID-19 Employee Symptom Attestation

FOR ALL EMPLOYEES working on-site at either Wenatchee Valley College campus

As on-site work restrictions are lifted by Governor Inslee, a daily COVID-19 symptom form will be required of employees returning to their on-site work location to ensure the safety and public health of the WVC campus and community. The form is required when individuals come to their work-site for any reason and for any length of time. 

These requirements apply to all faculty, staff and student employees who work on-site at a WVC work location including contract covered, classified non-union, temporary hourly, professional staff and exempt employees. Individuals who have a contingent worker relationship with WVC, stipend recipients, volunteers, and others who work on-site at WVC, but are not employees, are also subject to these requirements. Employees who are teleworking do not need to complete an COVID-19 Symptom form if working remotely at home or a non-WVC location.

Requirements for employees working on-site

Employees who work on-site at a WVC work location are required to:

  • Fill out the COVID-19 Symptom form that they are symptom-free each scheduled work day.
  • If individuals are experiencing symptoms and therefore do not or are unable to complete the form, they must stay away from their WVC work location.
  • Follow their department’s normal notification process to either take the day off or work from home when ill, as applicable.

Whether individuals are working on- or off-site, all employees must:

  • Contact the human resources office, (Tim Marker tmarker@wvc.edu or Reagan Bellamy rbellamy@wvc.edu) if:
    • diagnosed with COVID-19;
    • a healthcare provider instructs the employee to self-isolate because they are experiencing COVID-19 symptoms;
    • they experience COVID-19 like symptoms; and/or
    • they have been in close contact with someone who has COVID-19 (even if not experiencing symptoms).
Completing the COVID-19 Symptom Form

On each scheduled work day, employees who are expected to report to work at a WVC facility must complete an form to confirm that they are not experiencing any of the symptoms outlined in the form. The form should be completed before coming to work, however the time to complete the form will not be considered work time unless it is done at their on-site work location. Employees without technology available at home to complete the form before coming to work should work with their department to identify the best means for completion.

Experiencing symptoms

Employees who are unable to complete the attestation because they are experiencing symptoms should do the following:

If the employee is at home, the employee must:

  1. Remain at home and not go to work when sick, even if the symptoms are mild.
  2. Notify their supervisor according to their department’s procedures.
  3. Contact their health care provider by phone.

If the employee is already at work, the employee must:

  1. Inform their supervisor that they are experiencing COVID-19 symptoms, leave work, and go home.
  2. Contact their health care provider by phone.

Whether the employee is at home or at work, employees need to contact human resources (Tim Marker tmarker@wvc.edu or Reagan Bellamy rbellamy@wvc.edu) if experiencing the symptoms in the form. This ensures appropriate contact tracing and that employees are provided support in accordance with privacy regulations.

Employees may return to work on-site once they can attest that they are not experiencing symptoms and have notified their supervisor that they are able to return. Campus faculty, staff and student employees who have had COVID-19 may return to work on-site per their health care provider (a return to work note is required). Other ill employees may return to work on-site 72 hours after fever ends, without the use of fever-reducing medications. 

Information for Supervisors

Employees unable to complete the form due to symptoms

If an employee directly informs you they are experiencing COVID-19 symptoms or do not complete the form because they are experiencing symptoms, instruct them to:

  1. Go home, remain at home, or work from home.
  2. Request appropriate time off if not teleworking (contact Tim Marker tmarker@wvc.edu for help with appropriate leave).
  3. Contact their health care provider by phone.
  4. Contact human resources (Tim Marker or Reagan Bellamy).

If you receive information that one of your employees is exhibiting COVID-19 symptoms or you observe an employee exhibiting COVID-19 symptoms, discuss what was observed, and if the employee acknowledges having the symptoms, instruct the employee to go home and contact their healthcare provider, and contact human resources (Tim Marker tmarker@wvc.edu or Reagan Bellamy rbellamy@wvc.edu

If the employee provides a rationale for the symptoms, such as seasonal allergies, determine whether the response is reasonable in relation to what has been observed. Contact human resources (Tim Marker or Reagan Bellamy) if you need assistance making the determination whether this person should go home/work from home.

Monitoring the COVID-19 symptom forms

While the primary responsibility for adhering to the form requirements lies with each individual, supervisors with employees working on-site are expected to monitor form completion on a regular basis. They must follow-up with individuals who are expected to be working on-site, have not completed the form, and have not requested or been approved for any time off. Refusal to comply with requirements

These requirements are in place to comply with the direction of federal, state, and local authorities, and, more importantly, are in place for everyone’s health and safety. Supervisors should ensure all of their employees have a thorough understanding of these requirements. If an employee refuses to complete the attestation, the supervisor must:

  1. Direct the person to go/stay home.
    • Supervisors may consult with their supervisor and contact human resources (Reagan Bellamy rbellamy@wvc.edu).

Any individual’s refusal to complete the form will be treated as a performance or standard of conduct issue. Loss of pay may be in effect depending on the employee type and circumstances.
